Using the summary to outline the clinical follow-up activities does not make the best use of the available templates. It makes it difficult for an EHR to assist a provider in completing the tasks as they are not at the appropriate level of discreteness. We believe that using the plan of care template should be the long-term solution for identifying the necessary clinical follow-ups. This will allow an EHR to more easily consume that data and display it to an end user in an actionable format. This is not a small undertaking so we understand if there must be a short-term solution and agree that the summary makes sense. However, we think that the document should be forward thinking and include plan of care.
