It would be beneficial to include a discrete section (or entry) that points to a case number being maintained on the public health side.  That case number would be used for manual follow ups, and just generally to tie together information on the case without resulting in direct PHI exchange.  Since this may vary from jurisdiction to jurisdiction, including it in the jurisdiction-specific information in the Reportability Response would be best.
