Description
Background:
In the 2017 Errata for C-CDA R2.1, multiple errata changes were made affecting the Severity Observation template and the value set used by the Severity Observation template. The changes were part of a set of changes made to address issues in the Allergy - Intolerance Observation template reported by Patient Care.
Problem:
Changes made to the purpose statement of the Severity Observation template (CDA-834) now make the template not fit for purpose in its uses within QRDA Cat I. QRDA Cat I uses the Severity Observation more generally and may apply it to more than a Reaction.
Changes made to rename the Problem Severity value set to be the Reaction Severity value set (CDA-835, CDA-836) have compounded the "not fit for purpose" problem within QRDA Cat I.
Requested change:
1. Generalize the purpose statement of the Severity Observation template so it can be used to describe the severity of more than just an allergy or intolerance reaction. Add a caveat noting that the severity of problems may be precoordinated within the coding used to describe the problem.
2. Rename the value set bound to the Severity Observation value element to simply "Severity"
3. Loosen the binding stength for Severity Observation value from SHALL to SHOULD
