Support Sexual Orientation in case reporting spec

Several Public Health jurisdictions (including California) are interested in collecting SOGI (Sexual Orientation and Gender Identity) data. The eCR spec contains a template for Gender Identity but there doesn't seem to be a way to include Sexual Orientation. 

The Gender Harmony project may be working to add these concepts to all product families and should be involved in the development of an new CDA templates.
