      The Problem-Oriented Health Record project is part of the HL7 Reducing Clinician Burden (RCB) effort. Its core purpose is to identify how to evolve standards for a POHR. Central to the information contained in a POHR is the problem list, which is a necessary set of clinical data for clinicians providing care to a patient. It should identify known patient problems, not just problems of interest to a particular user. A complete problem list assures that (i) all problems are addressed and not over¬looked; (ii) all problems are taken into account when addressing any one problem. In these ways, a complete problem list facilitates comprehensive care and coordination of activities by multiple providers

      In addition to its core purpose of identifying patient problems, the problem list also serves as a navigation tool—a table of contents for rapidly seeing the whole record and accessing lower-level details on its parts.
