      Formatting

      • Numerous unnecessary section breaks that should just be page breaks are contributing to complex formatting challenges, many probably contributing to much of what follows.
      • Many occurrences of multiple margin changes on a page (e.g., page 17) and variations of indentation among like lines of a section or list of items (e.g., page 13).
      • Section 3.2 is not in the TOC. Page 11 is blank for some reason and is shown in the TOC as the first page of section 4. If I update the TOC, section 3.2 is added in a different font than other like sections, but page numbers are corrected.
      • Some paragraphs are left aligned while others are justified (aligned to both left and right margins).
      • Some paragraphs use double spaces between sentences while others do not.
      • Inconsistent use of the serial “Oxford” comma evident as soon as reading sections 1 & 2. (Comments made in the document.) This could be a conflict between HL7 and NCPDP preferred styles, but only one should be used.
      • Font color changes on some section numbers and titles (e.g. 3.2 and 5.2.x).
      • The tables in section 5.1 should probably be aligned to match indentation and width.
      • Section 5.2.1.3 follows section 5.2.4 and precedes section 5.2.5.
