      Remove DateTime template because:

      1. To represent the recorded time associated with the original data entry
        1. Add Author to the RecordedSexOrGender template
        2. Use AuthorTime constrained to a TimeStamp to record the original date-time entry
      2. To represent the clinical relevant time period of the recorded sex or recorded gender item
        1. Use effectiveTime low and high

      This correlates with the approach taken in FHIR extension for RSG
