Clarification of cardinality for Gender Identity and Pronouns

      Change each cell in the row, "Support zero to many instances", for GI and Pronouns to say: 

      "It is expected but not required, that there be only one gender identity value for any time period even though the genderIdentity extension/segment can be repeated."

      and

      "It is expected but not required, that there be only one pronoun value for any time period even though the pronoun extension/segment can be repeated."

      There will need to be additional V2 text that clarifies the constraints on LOINC codes in the GSP segment.

    Description

      Need clarificaiton that the entry in the comparison table for FHIR "For the instance (only one at a time/in the moment), there can be multiple over time" under Gender Identity, Pronouns and Sex for Clinical Use is correct.

      In V2 we allow mulitple repeats of the GSP segment (though that is used for any self-declared attribute, which is correct as it covers more than one of the attributes) - but we currently do not constrain the LOINC occurrence in the message for these elements - if that is not correct, we should fix it for GI and Pronouns.

      SFCU in V2 is using the GSC segment follwoing the patient and it includes the pointer to the related clinical context, so that segment needs to be able to repeat in the patient group also - not sure we have a mechanism that could limit the occurences here to match FHIR.
