Improve explanation avoidance of "code roll-up" in value sets

      In addition, it is important to note that in all value sets not bound Required, there is explicit guidance to NOT ROLL-UP other codes as "a kind of" one of the provided minimum codes. By this we mean that if a patient uses some other word, or concept, to represent the exchanged information, such as Gender Identity or Pronoun, the exchange should only provide the desired additional text or concept and not also "roll-up" to a code provided in the minimum set. Meanings in these domains can overlap and yet should always be considered distinct.

      For all value sets bound EXTENSIBLE or PREFERRED, implementers are discouraged from mapping to one of the included concepts other codes that do not exactly match in meaning.
