Care Plan by its definition should support referencing all of the other profiles in this IG to "bring them all together" creating an aggregated comprehensive view of the patients' physical activity Care Plan. I would not make it required to reference all - but I suggest that they all be Must Support from the care plan. Please also highlight where you are tracking the Outcomes (results of the Physical Activity Plan within the Care Plan (OutcomeReference)
