Social history does not seem to be an appropriate category for physical activity observations. Social history, as commonly understood, includes family, economics, education, opportunity, race, equity, community safety, food availability, and the like. 

Either remove the social history category, or make an argument on why Physical Activity is classified as "social history". Physical activity might be influenced by a person's social history, but it is not part of social history.
