In STU 2 ballot, the Risk Adjustment IG added an Annotation section to the Group element of the MeasureReport. This functionality allow a practitioner to add an annotation to the MeasureReport at point of care to indicate that they took an action against the gap, see http://build.fhir.org/ig/HL7/davinci-ra/report-annotation.html

This functionality would be important for Gaps in Care/Quality as well especially since it often requires ordering of labs, radiology, etc but it would be good to know an action was taken. This would also likely require some documentation update to tell how it is used similar to above.

Note: An annotation is just that, it does not close a gap, the Data Exchange MeasureReport would still be used for that purpose.
