      For FHIR Observations, the clinically relevant time is the time the assessment was made.

      Observation.effective[x] is defined as 

      The time or time-period the observed value is asserted as being true. For biological subjects - e.g. human patients - this is usually called the "physiologically relevant time". This is usually either the time of the procedure or of specimen collection, but very often the source of the date/time is not known, only the date/time itself.

      10.1.5.7 Physiologically Relevant Time of the Observation 

      The effectiveDateTime or effectivePeriod is the time that the observation is most relevant as an observation of the subject. For a biological subject (e.g. a human patient), this is the physiologically relevant time of the observation. In the case of an observation using a specimen, this represents the start and end of the specimen collection (e.g. 24-hour Urine Sodium), but if the collection time is sufficiently short, this is reported as a point in time value (e.g. normal venipuncture). In the case of an observation obtained directly from a subject (e.g. BP, Chest X-ray), this is the start and end time of the observation process, which again, is often reported as a single point in time.

      To wit SDC documents the mapping between Observation.effectiveDateTime to QuestionnaireResponse.authored.

      The “state or condition of being pregnant” is represented by the LOINC 82810-3 Description:

      This term should be used to indicate that the patient is currently pregnant, not pregnant, or that the pregnancy status is unknown at this time.

      The “intent to become pregnant” is represented by the LOINC 86645-9 Description:

      A patient's intention or desire in the next year to either become pregnant or prevent a future pregnancy

      The duration of intent or status is a separate question not represented by these profiles.

      States: Each observation must have 4. when the observation occurred. This is different to the C-CDA Pregnancy Observation in the C-CDA Companion Guide which has: "The effectiveTime element is used to describe dates associated with the patient's different pregnancy statuses over time. It represents the clinically relevant time of the observation about the patient’s pregnancy status.". Also the actual Description of the effectiveDateTime in the profile says "Clinically relevant time/time-period for observation", not when the observation occured which is in conflict with the description above. Consider updating to: "4. the clinically relevant time of the observation" and maybe adding a sentence in effectiveDateTime.definition like the above from the C-CDA companion guide.

      There are really 2 different dates involved - one is the date recorded and one is the date or dates that the Pregnancy Status is true. One should be a point in time and the other should allow for a start and end date.
