In https://jira.hl7.org/browse/FHIR-38199 and related tickets discussed on the 11/10/22 PAS call, we decided to move away from polling and instead make R5/R4B-style subscriptions a SHALL.

However FHIR versioning and dependencies make this difficult since http://hl7.org/fhir/us/davinci-pas/history.html uses FHIR 4.0.1, and the latest version of http://hl7.org/fhir/uv/subscriptions-backport/history.html uses FHIR 4.3.0.

We need to figure out what the subscription should look like, keeping in mind that PAS may become required through regulation.
