    Description

      Currently, the US Core Practitioner and Organization profiles only allow for a single NPI to be associated to each resource.  Unfortunately, the operational reality is that occasionally there are Practitioners and Organizations with whom there are multiple NPIs associated.  This was brought up separately in this zulip thread:

      Working with state programs, they similarly are tracking a few Practitioners and Organizations associated with multiple NPIs.

      To support these operational realities, we may want to consider relaxing the cardinality to allow 0..* NPI instead of 0..1
