      We are making several updates to the SFCU concept to mitigate some of these sorts of concerns, including renaming it from Sex for Clinical Use to Sex Parameters for Clinical Use.

      The FHIR extension definition (and name) will be updated to align with the new SBCU language.  

       

      New name:  Sex Parameter for Clinical Use

      New Description:  A Sex Parameter for Clinical Use is a parameter that provides guidance on how a recipient should apply settings or reference ranges that are derived from observable information such as an organ inventory, recent hormone lab tests, genetic testing, menstrual status, obstetric history, etc.  This property is intended for use in clinical decision making, and indicates that treatment or diagnostic tests should consider best practices associated with the relevant reference population.  When exchanging these concepts, refer to the guidance in the [Gender Harmony Implementation Guide](http://hl7.org/xprod/ig/uv/gender-harmony/).

      Some SGWG members felt strongly that this data element is discriminatory that promotes cisnormativity, implying anything outside of female and male need to be somehow specified; so we propose to vote NEGATIVE for this section. As an alternative, we suggest to change the label to Parameters for clinical use and separate it from Sex altogether, since other non-sex related clinical conditions may require special/specific considerations as well. Another suggestion is to use Organ inventory with a defined value set to remove having to make assumptions about sex status altogether.
