move all examples to standard IG format and fix resulting validation errors

Each of the event pages has it's own example(s). Eric points out in the linked issue that the imagingstudy-open example was conformant to R4. 

Each of these examples should be moved into it's own file and referenced in the ImplementationGuide resource, such that the IG Publisher validates it.

This will undoubtedly result in validation errors, which will need to be fixed.
