Change the .device description on the MedicationRequest and the MedicationAdministration to be the following (the bolded part can be the short definition):

This is the device that is an essential part of the administration of the medication.  A common example is a skin patch where a medication-containing adhesive patch is attached to the skin, and a specific dose of medication can be delivered to the blood through the skin.  The patch is not "performing" the administration but is rather the vehicle through which the administration is occurring.  Similarly, a syringe could be listed here if it was important to record the syringe as it is the means by which the medication is administered but the syringe is also not "performing" the administration.

Change the .performer description on the MedicationRequest and the MedicationAdminstration to be the following (the bolded part can be the short definition):

This is a device that is actually performing the administration of the medication.  An IV Pump would be an example of a device that is performing the administration.  Both the IV Pump and the practitioner that set the rate or bolus on the pump can be listed as performers.

    Description

We have the ability to specify the device on the request and administration which is currently "device intended to be used for the administration" as well as a device which is the performer which would be "device which is performing the administration".

We need to clarify when a device is "performing" the administration versus when it is just being used in the administration.

My initial take is that a device might be used in an administration but is not physically administering the medication so it would not be a performer.

Separately, I also believe that MedicationRequest.device should be 0..* and not 0..1 to support multiple devices that were used in the administration.
