    Description

      There's an invariant requiring that month and day always be present for Observation.effectiveDateTime but no comment explaining why.  The effectiveDateTime is when the observation is being asserted to have held - not the day the observation is captured.  It's not clear why there's a need to force the precision to always be to the day.  Either remove the constraint or provide the reason for it.
