      US Core is attempting to establish a floor for basic interoperability of clinical data no matter the source of that data.  Currently patient supplied data is not defined for US Core except for Clinical notes.

    Description

      There are a number of profiles in US Core that requires the data to be coming from a clinical source, like a Practitioner. Examples include CarePlan which requires an assessment plan category. DocumentReference which requires an Encounter as MS. And DocumentReference which requires a type of clinical-note. This does not allow for other use cases, particularly patient/consumer provided data, documents, or plans.
