      After consulting with the FM workgroup on 2/22/22, we have aligned on a solution to adjust the extension and move it from the header line to under the costToBeneficiary section.

      We will simplify the extension to remove the copay "type" section and present only the "cost" which contains the valueString. We will leverage the extension under the costToBeneficiary.valueMoney section. We will set this up as an invariant, to allow either a currency and value pair OR the string extension, not both.

      The explanation for the extension is:
      This extension allows for the representation of copay details as strings. This can be passed as part of the Coverage resource where payors need to communicate costToBeneficiary details that cannot be expressed as SimpleQuantity or Money data types.
