When an authorization comes back in an adaptive form, the SMART app needs to store that ClaimResponse on the EHR and update the "in-memory" request to have a supportingInfo reference to the ClaimResponse.  The first is done with a simple create.  The latter needs to be done using Smart Web Messaging
