Description
For patient profiles, I propose that we flag previous name and previous address with the "use" field, rather than utilizing the "period" field. This is small but both fields are called for by USCDI – I'd like us to standardize on how we document previous names and addresses.
Currently, the US Core Patient profile contains the following text:
"Previous name is represented by providing an end date in the Patient.name.period element for a previous name."
However, the period element utilizes a start and end date. As discussed in this chat thread , we may not know the actual end date for a patient's previous legal name. Even the patient might not remember. It seems like a bad idea to require use of the period element when we may not have the required level of specificity.
Instead, I propose that we utilize the name.use element, and set this to "old" for previous names. This can be done easily, and we do not require an end date for this.
In addition, I propose we do the same thing for the Previous Address – this was not specified in the notes but we could also use the address.use element, and set this to "old. Similar to previous names, we are unlikely to have the actual end date for when a patient moved from a previous address.
So the existing bullet point would be replaced with
- "Previous name is represented by setting the Patient.name.use element to "old"
- "Previous address is represented by setting the Patient.address.use element to "old"
This seems much simpler; and for those who know the actual end date they can still fill that out optionally.
I also suggest we include these notes in the USCDI to US Core mapping page. I found that page very helpful, but feel that if we do not include these notes there, then people will not see these instructions unless they read through the patient profile page carefully.
