We will add Not Applicable code to http://hl7.org/fhir/us/carin-bb/ValueSet/AMACPTCMSHCPCSProcedureCodes to be used by the Professional & NonClinician Eob.item.productOrService, and to http://hl7.org/fhir/us/carin-bb/ValueSet/ADADentalProcedureCode for Oral EoB.item.productOrService.

Also we will add an invariant to Professional & NonClinician EoB to disallow the use of not-applicable if the EoB.type is not vision.
