Description
Extensions consistently require (cardinality 1..1) the url field, and have an optional (0..1) value. An extension without the value is meaningless. So, like USCore, extension values should be required. Missing data should be handled by an overall missing data policy, a la https://www.hl7.org/fhir/us/core/general-guidance.html#missing-data .
