Lloyd McKenzie has indicated that the "instantiates" data elements have been removed from the Event and Request patterns and have been replaced by a series of "workflow'" standard extensions which fulfill the same purposes. Immunization and ImmunizationRecommendation are allowed contexts for several of these extensions and should be sufficient. The .instantiatesCanoncial and .instantiatesUri elements will be removed from both Immunization and ImmunizationRecommendation. Given that these elements were only added in R5, this change should not be problematic.
