      Bulk Data IG STU1 uses a search url parameter pattern for the kickoff request and did not support use of POST requests with a Parameters resource. To maintain backward compatibility, the IG supports both repeating parameters and/or including comma delimited values within a single parameter.

      1. Update  _type, _elements, _typeFilter, and includeAssociatedData so cardinality is 0..* in all three of our operation definitions
      2. We should add a note like "We might consider deprecating the comma-separate option in a future release."

    Description

      The approach to comma-separate multiple values differs from the definition for multiple values as defined by FHIR (https://www.hl7.org/fhir/operations.html#request ).

      Multiple values are to presented as multiple paramter fields in the Parameter resource or by repeating the named parameter.

      The spec must be revises and conform the FHIR approach.
