Add NamingSystem extension to support Identifier System sub-type

      We will:

      (1) Remove the current extensible binding of 'type' to the IdentifierType value set.

      (2) Create a new value set IdentifierSystemType that is defined as "all codes" from the http://terminology.hl7.org/CodeSystem/v2-0203 code system.
      Note: This is being done since the IdentifierType vs is also used for the Identifier data type.

      (3) Bind the 'type' element as 'preferred' to the IdentifierSystemType value set.

    Description

      Identifier Systems are moving from FHIR Core to THO.  Identifier Systems are commonly further qualified with a type, as is demonstrated with the CodeSystem found here:  https://www.hl7.org/fhir/v2/0203/index.html

      This extension will hold the value of one of the concepts in this CodeSystem. 

      The extension name is 

      namingsystem-type and should be bound with required binding strength to this value set
