Need for multiple comments in FHIR appointment

      There is an approved/un-applied change to update the patientInstruction to support multiple cardinality (the change also updates the datatype to CodeableReference permitting references to documentation) (https://jira.hl7.org/browse/FHIR-27849)

      Also note that the comment field on the Appointment is being renamed to note, datatype changed to Annotation, and support multiple entries as per approved change request (https://jira.hl7.org/browse/FHIR-19521)

    Description

      I am working on V2 to FHIR of SIU. SIU has multiple NTE segments following different parts of the segments. However the FHIR appointment resource only has one comment allowed in the resource. Is it possible to make it multiple?
