For R5 this valueset was updated to be an indicative set of values, not an exhaustive set. This was due to most environments/jurisdications having their own sets of values here, however we grabbed some commonly used ones used from v2 and v3.

(In R4 this was bound to a very large set 117 which was still inadequate with multiple requests)

Add substitute decision maker to the PatientRelationshipType http://hl7.org/fhir/valueset-relatedperson-relationshiptype.html. This is needed to identify who was approached to obtain consent.
