      We will consider the lessons learned by the Camden Coalition and the Chicago work as part of the overall industry experience related to working with social risk and integration into provider workflow and EHR and other repository representations of the associated information.

      Description

      Proposed reference for SDOH results from Camden Coallition for review to capture early work in this space and to consider the information flow external to the healthcare system into EHRs and other data stores that can aggregate and add the information to improve the health outcomes of the population (and person specifically) Dr Lindau has also done groundbraking work in Chicago supporting disadvantaged citizens by identifying and connecting patients to resources so would be another candidate for practical input for this community.
