Support qualified identifiers for models, in the same way that they are supported for libraries.

In particular, the qualifiers for a model identifier are the namespace, and they correspond to the namespaceUri for the globally unique model identifier.

When a qualifier is not provided, the default qualifier defined by the model info known to the translator is used.
