Such an observation is meaningless in isolation and should be a component, if it can't be accommodated with CIMI extension associatedSituationExt or with Observation.method.

Make clothing state a method, component, or extension (VS ~ https://build.fhir.org/ig/HL7/cimi-vital-signs/ValueSet-AssociatedSituationVS.html)

Existing Wording:

8352-7 - Clothing worn during measure and 8361-8 - Body position with respect to gravity MAY be included as additional observation codes.
