add a DragonBox to the R4B ConceptMap documentation with:

a link to the R5 CI build ConceptMap page

an alert letting implementers know that ConceptMap is changing

create a link to the structure map in the R5 draft that describes how to transform from R4 to R5 ConceptMap (to be created by rhausam); will be a new ticket.
