sdoh-clinicalcare has created profile on Observation to record the results of a question and answer from an assessment for social determinants of health.  We are building the profile on the base resource since there is no existing general observation profile in US Core for R4.  The current profiles are focused specific observations 

• US Core Laboratory Result Observation Profile
• US Core Pediatric BMI for Age Observation Profile
• US Core Pediatric Weight for Height Observation Profile
• US Core Smoking Status Observation Profile

• or the Vital Signs Profile from the FHIR Specification.

All of these profiles have required constraints and/or value sets that are not appropriate for the SDOH observations

We request an exception to the requirement to use a US Core Profile for this situation.
