Reason.reference element is needed to be able to be used within Activity when using activity outcome reference

      In R5, CarePlan.activity.detail.reasonReference (inline activity definition) has been removed, so this Jira is no longer applicable in R5.

      When referencing a planned activity resource, several resources do support a reason reference.  For example, MedicationRequest.reason (the request pattern includes a 'reason' element).  If you find any other resources that are missing a reason element, a Jira can be logged to the work group that owns that specific resource.

      In R4, you can use a contained CarePlan.activity.reference and the same guidance in the paragraph above applies.

      Need: ActivityReasonReference  **  in addition to ActivitydetailReasonReference.

      We need to be able to explicitly assert the problem or health concern for which an activity is being performed.

      Without the activity reason ALSO within the context of the Activity when using reference, the Care Planning process does not seem to be represented in the Care Plan
