      Rename: author to custodian
      Update Definition of custodian: When populated, the custodian is responsible for the care plan. The care plan is attributed to the custodian.
      Update Comments of custodian**; The custodian may or may not be a contributor.

      Update Definition of contributor: Identifies the individual(s), organization, or device who provided the contents of the care plan.

    Description

      Care Plan Author definition is over loaded in Care Plan: "Who is the designated responsible party". Definition for author seems as if it should be: Who provided the INITIAL content of the care plan. Which would align with the definition for Contributor "Who provided the content of the care plan" which maybe should be "Who contributed additional content of the care plan". Note that the ELTSS guide changed CarePlan.author definition to, "The person (e.g., Case Manager, Care Coordinator, Plan Coordinator) who helped develop the plan."

      I suggest that a CarePlan "Custodian" be added and align with the concept of  "Custodian" in Provenance and or C-CDA on FHIR:

      FHIR Provenance: "The entity that is accountable for maintaining a true an accurate copy of the original record" (Don't really like "original record")

      C-CDA on FHIR: "Identifies the organization or group who is responsible for ongoing maintenance of and access to the (composition/document) information."
