      Change the populationReference extension value from CodableConcept (with binding to MeasurePopulationType value set) to type of string, which will reference Measure.group.population.id in a Measure resource.

      Specifically document that a single resource contributing to more than one criteria may have multiple criteria reference extensions:

      Note that as many extension-populationReference elements can be added to a single resource as necessary. For example, if a single resource contributes to both the "initial population" and the "denominator", you can repeat the extension-populationReference. One repeat would have a "value" of Measure.group.population.id for the initial population and the second repeat a "value" of Measure.group.population.id for the denominator

      Also add a reference in the following section of the profile to the individual measure report profile:

      Through discussions at the connectathon, it was discovered that we do not clearly explain how to use the extension on EvaluatedResources.  For example if a single Resource contributes to both the "initial population" and "denominator", then it should contain two
