      Add notes to _query that search parameters except those specified in general search parameters table should be declared as part of the OperationDefinition. 

      Clarify that the name in "_query=name" is the operation's code defined in OperationDefintion

      Operations for _query search should not be invoked with $code operation

    Description

      It's not clear from this section whether you can use _include, _revinclude, etc. with a _query.  If they can be specified, do they need to be explicitly declared as part of the operation definition?  Also, we didn't explicitly state what the 'name' should be in `_query=name`.  Is it the OperationDefinition.code?  Is it legal to invoke such an operation using $code too?
