      When you have multiple RXR segments for an ORC/RXO, our guidance is to create multiple MedicationRequest instances.   The MedicationRequest.groupIdentifier would be used to group them together and both instances would have an MedicationRequest.intent of 'option'.

      We are going to create some examples to show how this would work and clarify the wording in MedicationRequest.dosageInstruction to reflect this guidance.

    Description

      In v2 upon ordering one can have 1 or more RXR segment for an ORC/RXO pair.  RXR would map to MedicationRequest.dosageInstruction.  The Dosage datatype includes attributes from both the RXR and the ORC/RXO pair.  

      When RXR repeats, it is not clear whether one is to create multiple MedicationRequest.dosageInstruction instances.  It appears that way, but four issues come up that need clarity.

      1) If you do, is one required to repeat the ORC/RXO information in each instance along with the RXR information?

      2) How does one indicate that the instances are options (either/or) vs. and-s?

      3) The current uses an example of options, but actually involves different medications where having separate MedicationRequests make sense.  But is not clear when the medication is the same (as is the case in the v2 example.  What is the proper guidance?

      4) While at it, if there are two MedicationRequests as alternatives, how are they linked as alternatives rather than two independent requests?  Would be helpful to explain somewhere.
