    Description

We find that this seems excessive – and how would you use this to calculate a quality measure? Are these for risk adjustment? Will the consumer aggregate all the problems/medications for the patients in the denominator and somehow calculate risk adjustment? This vague statement opens the door to consumers receiving much more information than is needed for quality measure reporting.

      Existing Wording:

How much data should be sent: (such as the complete problem or medication list ).
