      1. Rename:  activity.outcome to performedActivity (CodeableReference)

      Definition:  Identifies the activity that was performed. For example, an activity could be patient education, exercise, or a medication administration. The reference to an "event" resource, such as Procedure or Encounter or Observation, represents the activity that was performed. The requested activity can be conveyed using CarePlan.activity.plannedActivityDetail OR using the CarePlan.activity.plannedActivityReference (a reference to a “request” resource).

      Comments: Note that this should not duplicate the activity status (e.g. completed or in progress). The activity performed is independent of the outcome of the related goal(s). For example, if the goal is to achieve a target body weight of 150 lbs and an activity is defined to exercise, then the activity performed could be amount and intensity of exercise performed whereas the goal outcome is an observation for the actual body weight measured.

      Discussion:  "activity" is overloaded and represents both the request and event.  Renaming it to performed might imply a boolean (completed or not).

      Discussion: CodeableReference allows either CodeableConcept | Reference(Any).  The text will guide the reference to an event resource.  Event resource may have a status of not-done or in-progress.

      2. Rename:  activity.reference to plannedActivityReference

      Description: Activity that is intended to be part of the care plan.

      Definition: The details of the proposed activity represented in a specific resource.

      Comments:  Standard extension exists (resource-pertainsToGoal) that allows goals to be referenced from any of the referenced resources in CarePlan.activity.reference.
      The goal should be visible when the resource referenced by CarePlan.activity.plannedActivityReference is viewed independently from the CarePlan. Requests that are pointed to by a CarePlan using this element should not point to this CarePlan using the "basedOn" element. i.e. Requests that are part of a CarePlan are not "based on" the CarePlan.

      Description: In-line definition of activity

      Definition:  A simple summary of a planned activity suitable for a general care plan system (e.g. form driven) that doesn't know about specific resources such as procedure etc.

    Description

      CarePlan.activity.outcome element has the following description/definition statement:

      • "Results of the activity" (description), "For example, the outcome of an education activity could be patient understands (or not)" (example)

      This statement and example easily cause confusion with "Goal.outcome".

      CarePlan.Activity" data element is intended to specify (propose, recommend) the type of activities that are requested/ordered to meet the set/agreed goal targets. For example, if the goal target is for the patient to lose 20 lbs body weight in 5 weeks, the activity proposed/requested is for the patient to perform 30 min high intensity exercises 3x/week.

      There should be a data element in CarePlan to record what level of intensity, duration and frequency of activities the patient has performed, e.g. medium intensity exercises 45 min 4x/week

      Suggest change CarePlan.activity.outcome to CarePlan.activity.performed and revise the definition and example to rectify the confusion with Goal.outcome
