      The change request is to make the "code" binding extensible versus the current state of required. Currently, there are 3 codes available for recommendations and it seems highly unlikely these will be robust enough to serve the implementations yet to occur.

      Making the code list extensible will allow implenters to code whatever action is necessary including straight "text" when no code is available.

      I would also suggest reconsidering whether code should be required in this profile. In the event that folks want to pass along recommended actions as a text blurb without a specific coding it would be nice to be able to do it with this profile. However, it this profile is meant to only support coded recommendations then I suppose implementers with less structured recommendations would have to find alternative methods for sharing the recommended text blurbs often found on clinical documents. I do think that additional notes or documentation should be added to this profile to guide implementers as they would logically look to this profile for these less structured cases.
