      Fix the SNOMED to ICD-10 mapping example.  Both target concepts should be mapped from the source concept.  Also update the comments to accurately reflect how the additional encounter data affects the mapping to ICD-10 (ideally this should be included in 'dependsOn').

    Description

      During the 10/3/2019 Vocab FHIR Tracker call it was noticed that at least the two administrative gender concept map examples are not correct.

      Note: the definition of how to value relationship type code is under consideration and will be resolved with tracker 16364. Even if the definition stays as is documented in the current build of 10/3/2019, the relationship type code is incorrect in at least some of the examples. See tracker 24899.

      This ticket acknowledges the task to review. The review cannot be completed until 16364 is resolved.
