Explain how to have a Provenance reference elements within a target given GF#21284

      Add to 6.3.5.2 (http://build.fhir.org/provenance.html#using) to the notes about use of the extensions for targetElement and targetPath.

      The Provenance.target may point at a specific [element](extension-targetelement.html) or [path](extension-targetpath.html) within the .target reference. For example when updating a Patient resource with a new official name, such as shown in [example 1](provenance-example1.html), where the Provenance is indicating that the official name was provided by the Patient themselves.

    Description

      Given that GF#21284 is defining two extensions to support Reference that point not only at a Resource but specifically to elemnents within a Reference, add narrative on the Provenance page and an Example showing how this would work.

      Example is tracking who changed specific elements within a Practitioner resource.
