      Intended Recipient is a transport function. In XDS/XDR this is handled by the DocumentManifest. DocumentReference is metadata about the document, not about the transport. DocumentManifest includes a recipient element. Other solutions for delivery are available in FHIR model.

      Add a field to the resource to indicate the intendedRecipient (with the meaning used in XDS/XCA/Direct) which could either be a practitioner, patient, relatedPerson or ContactPoint

      intendedRecipient 0..* BackboneElement

      contact 0..* ContactPoint

      reference 0..* Reference(Practitioner|Organization|RelatedPerson|Patient)

      Constraint: At least one of contact or reference must be present.

      Consider adding a search parameter of type recipient-telecom (maps to intendedRecipient.contact) and recipient (intendedRecipient.reference)
