Change cardinality for Measure.name and/or Measure.title

      Name and title have specific expected display contexts that would be complicated by introducing multiples. Especially Name, as it is expected to be a machine-readable identifier for computable contexts.

      Further, the name and title elements are based on the [Definition](http://hl7.org/fhir/definition.html) pattern in FHIR, which defines both these elements as 0..1.

      Suggest using the `subtitle` element and/or providing specific use cases to help inform the proposed change.
