In MedicationRequest as part of addressing Reported, we added a Report[x] with a reportedBoolean and reportedReference, and removed the InformationSource element. This fix will:

Remove reported[x] choice

Leave 'reportedBoolean' as a stand alone element, with cardinality 0..1

Remove 'reportedReference

Add back in informationSource as a stand alone element with a choice of References (Patient, Practitioner, PractitionerRole, RelatedPerson, Organization) with a cardinality of 0..1 . Note that this is changed from previous informationSource with an addition of a new Reference - PractitionerRole.

This change supports both indicating the "reported reference" when a report is indicated, and also supported identifying the information source when there is No report indicated.
