I proposed either: 1) expanding the context one of the 2 scoring extension to Q.item and Q.item.option or 2) since scoring a Q seems like its is in the 80%: adding an inline element 'item.score' and item.score.option type Integer 0..1 with def: "Numeric values assiociated with answers for the purpose of tabulating scores" with notes about how this works with options etc.
