Description
Most (all?) of the CCDA extensions seem like they're CDA header elements rather than US-specific requirements. I'd like to see the following added as generic extensions on Composition rather than IG-specific extensions:
Data Enterer, Informant, Information Recipient, In Fulfillment of Order (but consider 'basedOn' and generify it to encompass ReferralRequest and potentially other request resoruces too), Authorization (though consider naming it 'consent')
I'm not proposing moving Participant and Performer because they're not well defined and it's not clear that they're well-enough differentiated from other extensions to justify their existence as good practice elements to use elsewhere.
(Also, please beef up the descriptions of the extensions)
